Individual Healthcare Plan Form

Although Individual Healthcare Plans are likely to be helpful in the majority of cases, it is possible that not all children will require one.  For example, most children with asthma or anaphylaxis can be supported using the care plan provided by the child’s GP/Nurse, a separate Individual Healthcare Plan will not be needed.
